The Hospice Outcomes Patient Evaluation (HOPE) Pocket Guide contains Chapters 1-3 of the CMS HOPE Guidance Manual v1.0 (effective October 1, 2025). HOPE was developed by CMS to replace HIS and to help hospices understand patient care needs and to gather quality, patient-level data for HQRP requirements. This handy guide will fit into the clinician's bag so that they can easily access the HOPE guidance in the field and while completing their documentation.
